1、寫法
首先，應該按照半正式的格式在假條的上方還是寫上如下信息：

假條抬頭範例如下：

其次，在您的請假信第一段，應該開門見山但是有禮貌地提出請假。第一段要中心明確，寫清您要請假的日期。
