此雇用契約由＿＿（以下簡稱僱主）和＿＿（以下簡稱雇員）締結。
根據本契約，僱主將聘用雇員且雇員同意受聘於僱主就以下所規定的工作提供服務和履行義務。
A.義務和責任：契約雙方同意雇員將受聘從事＿＿工作，並履行以下職責。
B.期限：本契約有效期為＿＿（年、月），從＿＿年＿＿月＿＿日起至＿＿年＿＿月＿＿日止。

F.主要工作地：雇員主要工作地為＿＿。但根據僱主業務性質所需，在不違犯勞工政策和規定情況下，僱主也可要求雇員在＿＿地以內其他選區履行職責。
G.差旅費：在本契約到期或中止（不論中止原因如何）時，僱主將負責雇員返回僱傭地的機票費用。
H.保險和醫療費：僱主應負責雇員的醫療保險或負擔雇員的全部醫療費用，包括＿＿地以外的轉診和轉院費，如雇員因故死亡，僱主應承擔屍體保存及運回原地的費用。
I.最近的血親的通知：雇員在出現重病或死亡情況，僱主應立即通知其最近的血親，雇員最近的血親的姓名和地址如下：

J.食宿及其他：僱主必須保證提供以下方便：

L.終止契約：本契約可根據下列規定終止：
1.無故：由一方當事人提前＿＿天以書面通知另一方當事人；或
2.因故：在工長或其代理人力圖調解爭議失敗後，由一方當事人提前＿＿天以書面通知另一方當事人。
（A）因故解除契約時，僱主應支付雇員契約解除生效前的薪金，並為雇員購買他（她）返回僱傭地的單程機票。
（B）下列任何一條均構成契約終止理由：
a.雇員＿＿次無故曠工和（或）＿＿次無故上班遲到；
b.雇員行為疏忽魯莽，或不完成任務；
c.在＿＿地犯下重罪或兩項或兩項以上輕罪；
d雇員放棄工作職責；
e.不稱職或在資格、技術、身體和精神方面與所填報情況不符，無法履行僱傭規定的職責；
f.在物質或其他方面受到特別虐待；
g.無故拖延支付雇員的工資；
h.違犯本契約任何一項規定；

M.爭議的解決：
怨情和調解
因本契約而產生的所有怨情與爭議均應按以下程式解決：
1.雇員須就雇用而產生的一切怨情或爭議向其管理員報告，如管理員不在，可直接報告僱主；
2.如管理員不能立即解決問題，管理員應將怨情或爭議寫成書面材料，同時寫明被觸犯的契約段落，法律或規章制度。管理人員應在五天內解決怨情或爭議，或寫出其認為沒有違犯規則的理由。

五、乙方在受聘(雇)用期間，除應遵守甲方一切行政法規之規定外，其假期、離職儲金給與、勞工保險及全民健保等，依左列約定：
