④ 組織機構代碼證和營業執照副本的複印件，加蓋公章；

⑤ 就業許可證的複印件；

⑥ 代辦人的身份證複印件，加蓋公章。

辦理期限：3~5個工作日（主管出差較多，需提前提交）
